Required forms

Please complete all required registration and waivers before your scheduled appointment. Select the form that applies to your visit and sign electronically.
New clients beginning individual training must complete both the New Client Registration and an Individual Session Waiver.

Required before each evaluation or separately scheduled testing appointment.

Required for each dog before beginning individual training or a training package.

Package clients must complete a new registration at the start of each package, with annual renewal when applicable.

Required before EVERY individual training appointment.

New clients must also complete the New Client Registration before their first session.

Required before EVERY lifetime maintenance appointment.
